1. About Us

St. Clair Drug Mart Pharmacy (“the Pharmacy”) is located at 1203 St. Clair Avenue West, Toronto, ON M6E 1B6. We are a health information custodian as defined under the Personal Health Information Protection Act, 2004 (PHIPA). Our Designated Manager and Privacy Officer is Elizabeth Da Silva, BScPharm, RPh. 2. What This Policy Covers

This policy applies to personal health information (PHI) and other personal information we collect through this website and in the course of providing pharmacy services. It does not govern information collected directly by third-party platforms (see Section 6).

3. Information We Collect

• Pharmacy services: Name, date of birth, health card number, medication history, prescriptions, and other PHI necessary to dispense medications and provide clinical services.
• Drug Interaction Checker: Medication names you enter are sent anonymously to the U.S. National Library of Medicine (NLM RxNav API). No name, health card, or identifying information is sent. This website does not store any information you enter in the checker.
• Contact & inquiries: Name, email, and phone number when you contact us voluntarily.
• Website usage: This website uses Google Analytics to understand how visitors use the site — such as which pages are visited, how visitors found us, and general device and location information (city level). This involves analytics cookies. The data is aggregated and does not identify you personally, and no personal health information is ever collected or shared through analytics. We do not use advertising cookies. See Section 6 for details and how to opt out.

4. Purposes of Collection

We collect PHI only for the purposes of providing healthcare — dispensing prescriptions, clinical assessments, medication reviews, and pharmacist consultations — or as required by law. We do not sell, rent, or trade your personal information.

5. Legal Authority

Collection, use, and disclosure of PHI is governed by PHIPA (Ontario) and, where applicable, the Personal Information Protection and Electronic Documents Act (PIPEDA). We comply with Ontario College of Pharmacists standards for patient record retention and confidentiality.

6. Third-Party Platforms

PharmAssess: Online booking, refill requests, prescription transfers, and minor ailment assessments are processed through PharmAssess (Kroll Computer Systems Inc.), a secure pharmacy software platform. PharmAssess acts as a service provider to our pharmacy. As your health information custodian under PHIPA, St. Clair Drug Mart Pharmacy remains accountable for the protection of your personal health information. NLM RxNav: The Drug Interaction Checker sends anonymous drug names only to the U.S. National Library of Medicine public API. No identifying information is transmitted.

Google Analytics: This website uses Google Analytics 4, a web analytics service provided by Google LLC, to measure website traffic and improve our online services. Google Analytics uses cookies to collect information such as pages visited, time spent on the site, approximate location (city level), browser, and device type. This information is aggregated and does not identify you personally. No personal health information, names, health card numbers, or booking details are ever sent to Google — online bookings and health assessments take place on the separate, secure PharmAssess platform (see above), which is not covered by analytics. Data collected by Google Analytics may be processed on servers outside Canada, including in the United States. You can learn more in Google's Privacy Policy, and you can opt out at any time by using the Google Analytics Opt-out Browser Add-on or by blocking cookies in your browser settings — the website works fully without them.

7. Disclosure

We may disclose PHI without your consent only as permitted or required by PHIPA — for example, to another health information custodian involved in your circle of care, or as required by law. We will not disclose PHI to third parties for marketing purposes.

8. Security

We implement physical, administrative, and technical safeguards appropriate to the sensitivity of the information, including secure storage of patient records and restricted access by pharmacy staff.

9. Your Rights

• Request access to your PHI held by the Pharmacy.
• Request correction of inaccurate PHI.
• Withdraw consent where consent is the legal basis for collection or use.
• File a complaint with the Information and Privacy Commissioner of Ontario.
